Key Features of Er:YAG Fraxel Treatment

Utilizes Er:YAG Laser Instead of CO2 Laser:

CO2 lasers penetrate deeply but can cause strong thermal reactions in skin tissue, increasing the possibility of post-treatment hyperpigmentation.

In contrast, Er:YAG lasers are Fraxel devices that precisely deliver energy from the epidermis to the dermal-epidermal junction while minimizing thermal damage.

Adjusting Treatment Parameters Based on Individual Skin Condition:

Considering each patient's unique skin characteristics and areas of concern, we delicately adjust the laser's depth and intensity.

For thin skin, we perform multiple treatments with lower power, while for thicker areas, we apply single high-power, tailoring adjustments to minimize skin burden during treatment.

Precise Treatment Using Overlap Technique:

For specific areas requiring improvement, such as fine lines, scars, or enlarged pores, we apply an overlap technique, repeatedly irradiating the laser to precisely control treatment intensity.

It can be applied to improve fine lines on various areas beyond the face, such as the back of the hands or the neck. For body parts that may be challenging to treat, we carefully consider skin regeneration speed and sebaceous gland density to provide customized treatment settings.

Enhancing Fraxel Treatment Effects Through Skin Booster Delivery Technique:

Immediately after Fraxel treatment, microscopic channels are momentarily created in the skin, making it highly receptive to the penetration of active ingredients.

At this time, we apply a protocol of topical application of skin boosters to help active ingredients effectively reach the dermis.

By delicately adjusting the laser overlap and intensity according to the depth of scars or varying skin thickness, we create an environment where the active ingredients of skin boosters can be sufficiently absorbed up to the dermal-epidermal junction.

When combining with small-particle skin boosters like PDRN or Juvelook, this process allows for the absorption of ingredients into the dermis, which is crucial for skin regeneration. This can positively impact the improvement of pores, fine lines, and acne scars.

Understanding Fraxel Treatment

Fraxel is a concept derived from 'fractional,' meaning 'partial.' It refers to a method where, instead of irradiating the entire skin surface with a laser, energy is delivered only partially with microscopic spacing.

It works by creating tiny microscopic holes in the treated area, artificially stimulating the skin, which in turn activates the skin's natural regeneration process.

This method of inducing partial damage to the skin offers the advantage of expecting overall skin condition improvement while relatively reducing the burden of post-treatment recovery.

Difference Between Ablative and Non-Ablative Fraxel

Ablative Fraxel microscopically removes the epidermal layer and delivers energy to the dermal-epidermal junction. This can directly impact the improvement of skin texture, wrinkles, and pigmentation, and may require a certain recovery period after treatment.

Non-ablative Fraxel delivers thermal stimulation to the dermis while preserving the epidermis. The recovery period may be relatively shorter, but the expected treatment effects might be comparatively limited compared to ablative treatments.

The Er:YAG Fraxel used at our medical institution falls under ablative laser treatments.

Principle of Er:YAG Fraxel Treatment

Er:YAG Fraxel is an ablative laser characterized by a 2940nm wavelength, which has a high absorption rate in water.

This wavelength can precisely vaporize and remove tissue from the epidermis to the dermal-epidermal junction. This makes it easy to control treatment depth and minimizes thermal damage, offering the advantage of relatively faster recovery.

Since it delivers energy mediated by water within the tissue without causing excessive thermal stimulation to surrounding tissues, it can help reduce the possibility of post-treatment hyperpigmentation.

Comparison of DOE and MLA Methods

Fraxel laser energy delivery methods are broadly divided into DOE (Diffractive Optical Element) and MLA (Microlens Array) methods. The DOE method uniformly delivers energy across the entire treatment area and can distribute energy relatively safely. However, there may be limitations in power output, so the practitioner's skill is an important consideration for scar treatments.

In contrast, the MLA (Microlens Array) method concentrates laser energy strongly at the center, with intensity decreasing towards the periphery. This may be more suitable for improving deep scars, but excessive energy concentration in specific areas could potentially prolong the recovery period.

Expected Effects After Er:YAG Fraxel Treatment

Our medical institution's Er:YAG Fraxel utilizes the DOE method for laser irradiation.

This can help reduce the burden of treatment by delivering energy uniformly across the entire skin.

Therefore, the DOE method can positively impact the improvement of pores, fine lines, and skin texture. For areas of concern like acne scars, combining it with the overlap technique can lead to further improvement.

Collagen and Elastin Regeneration: The treatment removes damaged skin tissue and stimulates fibroblasts in the dermis to help reconstruct the skin to be thicker and firmer. This can contribute to improved skin texture and enhanced elasticity.

Pore Tightening Effect: After treatment, the fibrous tissue around the pores undergoes a rearrangement process, leading to a reduction in pore size and an overall refined feel of the skin surface.

Acne Scar Remodeling: Old scar tissue can be gradually broken down and replaced with new collagen tissue. Through this, a positive impact on improving skin texture caused by acne scars can be expected.

Research Data on Er:YAG Fraxel

Serdar ZA et al. Dermatol Ther. 2019 publication:

Research results from a study of 333 patients with facial and neck wrinkles.

Records show positive changes in skin texture, wrinkles, and pigmented lesions after 3-4 sessions of Erbium YAG Fraxel treatment.

(Source: Robati RM et al., J Cosmet Dermatol. 2020)

We have confirmed treatment cases for those who wish to improve facial pores and fine wrinkles.

Improvements in fine wrinkles could be expected with just one treatment session.

If you receive 3 or more treatments, you can expect the effects to last relatively longer and for pores to appear reduced.

A Message from the Head Director

Since each patient's skin condition varies daily, we always take this into consideration during consultations.

During Fraxel treatments, we carefully check your skin condition on the day of the procedure and meticulously adjust the laser output and penetration depth in 0.1 unit increments.

Our medical team always prioritizes precision and safety in patient treatments.

We believe that Fraxel treatment cannot be perfected with just good equipment; the skilled technique of the practitioner is equally important.

We will always strive to repay the trust of everyone who visits us.

Q&A

Q&A

고객님들이 가장 많이 물어보시는 내용을 모았습니다.

How much pain is involved during Erbium Fraxel treatment?

QnA

We apply an anesthetic cream before the procedure to reduce pain. You may feel a slight stinging sensation during the treatment, and after the procedure, your skin may be red or feel hot for a day or two.

When do scabs appear and disappear after the treatment?

QnA

Approximately 3-4 days after the treatment, fine dead skin cells and scabs will begin to appear on the skin. These scabs will naturally fall off starting after about a week, and although there are individual differences, most of them usually disappear within 1-2 weeks. Due to the peeling effect, temporary redness may appear on the treated area, which can also last for about 1-2 weeks.

How long can the redness last?

QnA

Post-treatment skin redness usually subsides within 1-2 weeks. However, due to individual differences in skin sensitivity, dryness, and overall skin health, the skin regeneration speed may vary, and redness may last longer than expected in some cases.

Is it okay to wash my face or apply makeup on the day of the treatment?

QnA

It is recommended to refrain from washing your face or applying makeup on the day of your Erbium Fraxel treatment. Gentle cleansing and light makeup are possible from the day after the procedure.

What is the recommended treatment interval?

QnA

Typically, we recommend 3 to 5 sessions at 4-week intervals. After that, continuing with maintenance treatments once every 3-6 months can consistently help improve your skin.

When can I expect to see the effects after the treatment?

QnA

Collagen production in the skin gradually begins 2-3 weeks after the treatment. After about a month, you may feel that your skin is more elastic. Treatment results vary individually, and multiple sessions may be required, especially for deep-seated concerns that have been present for a long time.
